Ambrose goes on to explain that the fugitive escaped the county jail by hitting a guard with an iron pipe. He also tells us why Orrin had escaped the jail; he escaped because he was awaiting trial. By Bierce telling the reader that Orrin was awaiting trial for murder and escapes, he is showing us that he is desperate and trying to escape his fate. It does not take Bierce long to create his world and he knows very well how to do it. A second story that demonstrates Ambrose Bierce’s world is â€Å"A Wireless Message†. This story really outlines the uncertainty and confusion that lives in the writing world of Ambrose. The story opens up with Bierce telling us that William Holt, the main character, has just recently moved to New York from Chicago in the last year. The main reason why William has moved was that him and his wife where having troubles. It’s not a bad reason but the way Bierce tells us this is how he starts to create his world. Ambrose simply just tells us â€Å"Mr. Holt had had trouble with his wife†. He uses this line to start to create his world by only giving us a little bit of information; he leaves us confused and uncertain. Bierce hadn’t told us how much his wife meant to him or what kind of troubles they were having, all he does is leave us with questions to be asked. Ambrose Bierce is good at creating confusion and uncertainty in his world and stories. The examples from the short stories of Ambrose Bierce are only the tip of the iceberg in how he creates his world as a writer. His world is always one that has despair or some sort of confusion and a great style that makes his stories very successful. According to Wikipedia (2006) â€Å"the street racers conduct their activities either during normal traffic or during empty hours of traffic. This is often happening within the industrial areas of cities. This form of racing is illegal such that driving is prohibited by many traffic regulations.†The law prohibits these races (McClurgs, 2001), chase and pursue the violators. â€Å"But because of the frequency of the illegal street racing, most of the time, the racers either are not caught or they get away (Wikipedia, 2006).† â€Å"Speeds in an illegal street race can reach over 200 miles per hour. Car crashes can and do occur (Lopez, 2001).†The most common form of street racing in the US is a drag race of modified stock cars. According to Wikipedia (2006), â€Å"this is usually held late at night on straight public roads with very low traffic that are often illegally closed by the race organizers.†Wikipedia (2006) further explained that â€Å"professional drag racers who race on sanctioned drag strips often object to the use of the term drag race to describe an event of this type. The motivations behind illegal street races are many.† â€Å"For instance, lack of a nearby drag strip, the temptation of illegality street racing which could be interrupted by a police squad any time, or just the similarity to a day to day situations at a traffic light are some of the motivations (McClurgs, 2001).†Street racing is also a growing business. These street racers are consumers of a billion dollar aftermarket tuning industry. They are also the big market in the automotive industry.   Street races can also occur on expressways and highways, like in Japan. The most notorious group to be associated with it was the MidNight Club who gave street racing a worldwide attention with its 198 mph (300 km/h) antics. The group was known for its high standards and or ganization. But in 1999, they were disbanded following a fatal accident involving a competing group of motorcyclists. Presently, with heavier punishments, patrolling police cars and speed cameras, expressway racing in Japan is not as common as it was during the 80s and 90s. However, street racing still occurs in Japan on a regular basis (McClurgs, 2001).The blackracers, is a legal street races that occurs in some places This is not the same as road racing. It is strictly an amateur sport with road legal vehicles. Usually the races are done on a closed road and run on time and not against another vehicle (Wikipedia, 2006). The most famous blackracer is the annual Silver State Classic. In Japan, racers slide around the corners of remote winding mountain passes (Wikipedia, 2006). This sports is called Touge. Touge is the origin of drifting (Lopez, 2001).Street races occur all over the United States. It is almost inevitable that street races will occur anywhere there are public roads. U nsanctioned racing with unknown participants on an unknown course for an unknown duration is the most common type of road racing sport. A contest may last seconds or many hours. The informal events have the common characteristic that the car in front at the finish wins, the leading car chooses the course, and the finish is when the stalking car quits.Some states have more active racing scenes than others. For instance, there is a strong racing culture in Southern California, which centers quite heavily around imports, as well as such American muscle cars as the Ford Mustang, Dodge Charger, and Chevrolet Camaro. There are also very active street racing cultures in New York, Florida, New Jersey, Nevada and Texas. In some cases, the popularity has led to tough anti-street racing laws which give more strict punishments than normal traffic citations and also often dedicated anti-racing task forces.In the year 2005, a law in Tennessee that was passed prohibited cars to have Nitrous Oxide hooked up or even present inside a car, among other performance enhancers. Penalties include impoundment, taking of driver’s license for a period of time or permanently, This came about after a fatal crash in Johnson City, Tennessee (Yates, 2004).The programs like RaceLegal.com, DragNet and others are borne because there is a problem. These programs are also created by people that care about not only the innocent victims who street racers run into, and kill, maim, or burn to death. Most of these programs are run by, and created by the law enforcement.Law enforcers are not out to harass street racers. It may be probable that a number of law enforcement officers would rather let the street racers do their own thing if these street racers would simply do it safely, and not endanger the general public.A number of these street racers have little or no regard for life. Some even have no driving experience. These racers see all the hype of street racing on television but some of the m do not have a clue that, they, do not even come close, to having the experience and expertise it takes to become a true racer. The true racer uses a legal track not the streets. I am in favor of a mandatory five years sentence for the street racers violators along with the same amount of time with a suspended license. The second offence should be a total life time ban of their driving privileges. Driving is a privilege, not a right, so as citizens we have the right to expect a safe commute to and from our destinations without having these street racers interrupting it or killing people (Smith, 1996).The law breakers are not being slap on the wrists and put them in community service but they are given a five year sentence for speeding in town. I do not think that that is harsh. It is true that there are more deaths by Drunk Driving than Street Racing. But at the rate this going it wont be long before Street Racing catches up. Some people think that only the people Street Racing get hurt or killed. It is not true. There have been many innocent lives lost to Street Racing.Street racing has been around since the first car rolled off the assembly line. Nowadays, the cars are faster and the drivers are younger. Places like Qualcomm and Carlsbad are helping to decrease street racing, but since they are not open at later hours, they cant do it all. Street racing will never cease. It is American as apple pie. By opening more places to race and/or keeping current tracks open later, the street racing will decrease more. Instead of persecuting everyone that has a fast looking car, people should try to come up with ways to help (Ryan, 1998).Street racing done on city streets is illegal. You can be hurt or killed or even hurt or kill someone else. These are not merely speculations that some might say could happen. They have happened and continue to happen everyday due to those people who think that it is cool or fun to race cars on the streets. Nationwide statistics have shown that almost fifty people in every one thousand who are participating in  Ã‚  Ã‚  Ã‚  Ã‚   street racing, are killed. That is a lot of young people that would be alive today if not for illegal street racing (Smith, 1996).If you are caught street racing, there are stiff penalties. Your can be arrested and spend as much as three months in jail, and your car be impounded for up to thirty days. You will also be charged with a fine of up to $1,000 while your driver’s license is revoked and your insurance can be cancelled. Onlookers are also fined and their cars impounded as well.Street racing is a big problem. Places to legally race should be a priority. Even if there are only a few places, at least there are choices on the places that the street racing should take place. The driving time is truly worth your life, and the life of anyone who might be hit while racing on a public street.For as long as these young people are merely trying to have fun it will be a good hobby. I t will be a good thing if one of these street racers could be the next Jimmie Johnson. It will be a good project if a racetrack will be built for all these young people to go race until their hearts content. It would keep street racing off the road and the anti-fun crowd would maybe stop their whining. Likewise, it would give the potential future Nascar stars a place to race and have fun.But at this period, the bottom line is that street racing, while it might seem like fun, it is illegal. Stubbornly continuing these illegal street racing may completely change your life in the short time that it takes to start up that car and take off. There are many proofs of these. Just turn on the television, listen to the radio, read newspapers, magazines, books, etc., there are enormous instances that will remind us how dangerous and destructive street racing is. You can even ask those who now speak out about the dangers of street racing from their hospital beds or their prison cells (Yates, 20 04).However, it is interesting to note that street racers do have options. They can race legally on the race tracks that provide them with a certain amount of safety and peace of mind. Think about these professional racers that are seen on television. These are the people that most of those involved in illegal street racing are trying to mimic. Yet, they have never seen these folks racing on a city street. Their racing takes place on a track and so should the others.Racings prevailing attitude in this country toward safety begins with the waiver. The basic premise of Bingo is simple: players start with a grid filled with answers and they cover up spaces as the corresponding item is called from the Bingo caller. Winners make a complete line going vertically, horizontally, or diagonally. Or, you can play Black Out which means the winner is the first person who covers all of the spots on the card. Preparation There are a few ways you can prepare for playing Bingo in your classroom. Buy a Bingo set from a teacher supply store. Of course, this is the easiest way, but we teachers don’t make too much money so this option may not make too much sense.A cheaper option requires you to prepare all of the Bingo boards ahead of time, making sure that all of the boards are configured differently from each other.For older students, you can hand over some of the preparation to them. Prepare one Bingo board with all of the options filled in. Also, keep a copy of a blank board. Make copies of each page, one per student. Give the children time to cut apart the pieces and paste them wherever they want on the blank boards.The most teacher-friendly way to do Bingo is to give each child a blank piece of paper and have them fold it into sixteenths. Then they get to write the terms into their bingo sheet from your list (on the chalkboard or overhead) and voila! Everyone has their own unique Bingo board! You can play Bingo with virtually any subject. Here is a rundown of some of the different ways you can play Bingo in your classroom: Language Arts Phonemic Awareness:  Kindergarten teachers can use this type of Bingo to help students learn the sounds that correspond to the letters of the alphabet. On the Bingo chart, place single letters in each of the boxes. Then, you call out the letter sounds and the students put a marker on the letter that  makes  each sound. Or, say a short word and ask the kids to identify the beginning sound. Vocabulary:  In the Bingo chart boxes, put the vocabulary words your class is currently studying. You will read out the definitions and the kids have to match them up. Example: You say to find and bring back and the students cover up retrieve. Parts of Speech:  Get creative with using Bingo to help kids remember the parts of speech. For instance, read a sentence and ask the kids to place a marker on the verb in that sentence. Or, ask the kids to look for a verb that begins with a g. Make sure there are all different types of words that begin with that letter so that they really have to think about it. Math Subtraction, Addition, Multiplication, Division:  Write the answers to applicable problems in the Bingo boxes. You call out the problem. This is a great way to reinforce the math facts that kids must memorize. For example, you say, 6 X 5 and the students cover 30 on their game sheets. Fractions:  In the Bingo boxes, draw various shapes cut into portions with some of the parts shaded. Example: draw a circle cut into fourths and shade one of the fourths. When you read out the words one fourth, the students will have to determine which shape represents that fraction. Decimals:  Write the decimals in the boxes and call out the words. For example, you say, forty  three  hundredths and the kids cover the square with .43. Rounding:  For example, you say, Round 143 to the nearest 10. The students put a marker on 140. You may want to write the numbers on the board instead of just saying them. Place Value:  For example, you say, place a marker on a number that has six in the hundreds spot. Or, you can put a large number on the board and ask the students to place a marker on the digit that is in the thousands place, etc. Science, Social Studies, and more! Vocabulary:  Similar to the vocabulary game described above, you say the definition of a word from your unit of study. The children place a marker on the corresponding word. POEMS Syndrome Symptoms and Treatment POEMS Syndrome: Paraproteinemic neuropathies, Organomegaly, Endocrinopathy, M-protein and Skin changes Abstract The POEMS syndrome, also known as Crow-Fukase syndrome, is a rare multi organ disorder characterized by polyneuropathy, organomegaly, endocrinopathy, M-protein spike and skin changes. Other associated features, such as sclerotic bone lesions, edema, ascites, hematological disorÂÂ ­ders and Castleman disease can also be present. We report a case of POEMS syndrome who presented with insidious onset, progressive sensorimotor polyneuropathy, pedal edema, ascites, hepatomegaly along with skin changes. X-ray pelvis showed osteosclerotic lesions. Thyroid function tests showed hypothyroidism. M-protein (IgG) monoclonal band was seen on immunoelectrophoresis. The patient was started on melphalan and corticosteroid combination therapy. We emphasis on the importance of recognizing a challenging diagnosis of a rare disease, which is shown to be treatment responsive. Introduction POEMS syndrome is a rare paraneoplastic disorder of plasma cell dyscrasias, which was first described in 1956 by Crow and then in 1968 by Fukase [1]. The name POEMS was given to it by Bardwick and co-workers in 1980 based on five salient features: polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy and skin changes [1]. It is more prevalent in men, with male to female ratio of 2.5:1. It usually manifests in 5th and 6th decades of life. Its inheritance is uncertain and its pathophysiology is still not well understood. Case Presentation A 40-year-old male presented with progressive weakness, tingling and numbness sensation in both lower limbs for two years. He had swelling of lower limbs, abdominal distention and dermatologic changes in form of discoloration and thickening of skin over the cheeks, nose, hands and feet for one year. He had also gave history of erectile dysfunction and loss of libido for six months. There was no history of syncope, bony pain or drug abuse. He had no previous history of hypertension, diabetes or tuberculosis. On physical examination, the patient had bilateral pedal edema [Figure 1.c] and abdominal distention [Figure 2.a]. Skin was thickened and hyperpigmented over the face, fingers of the hands and shin (Figure.1a,b,c). Bilateral gynecomastia and testicular atrophy were present. Abdominal examination showed hepatomegaly and ascites. Higher mental functions and speech were normal. Fundus examination showed papilledema on both side and rest of the cranial nerves examination were normal. Motor power in upper limbs was normal and in lower limbs showed predominant distal weakness [Medical Research Council (MRC) 4/5 at hip joint and 4-/5 at ankle joint). Deep tendon reflexes in upper limb were diminished (+1) including biceps, triceps and supinator and absent in lower limbs. There was 30% loss in pain, touch and temperature sense in both lower limbs below knees. Posterior column sensations (joint position and vibration sense) were also impaired in lower limbs below the anterior superior iliac s pine. Romberg sign was positive. Hemogram, liver and renal function tests, muscle enzymes (creatine phosphokinase), serum ferritin and vitamin B12 level were normal. Serum total protein was 7.1 gm/dL, albumin 3 gm/dL, globulin 4.1 gm/dl, and A:G ratio 1:1.3. Fasting and postprandial blood sugar level were normal. Thyroid function test showed raised TSH level (16.62 ÃŽ ¼/ml). Luteinizing hormone (LH) and testosterone levels were 15 IU/L (1.8-8.6 IU/L) and 111 ng/L (300-1,000 ng/dL), respectively. Antinuclear antibody (ANA), Rheumatoid factor (RF), serum human immunodeficiency virus (HIV) ELISA test, hepatitis B and C markers were negative. Ultrasonography of abdomen showed hepatomegaly (16 cm), moderate ascites and enlargement of multiple lymph nodes along the iliac vessels. Fine needle aspiration cytology of mesenteric lymph node was inconclusive. Ascitic fluid examination revealed exudative nature (SAAG Discussion POEMS syndrome is a rare, multiple system disorder, characterized by polyneuropathy, organomegaly, endocrinopathy, monoclonal or M-protein band and skin changes. Any three of the five features may be present to establish diagnosis [2]. However, some authors have proposed clinical criteria for diagnosis in which includes two major criteria, which can be either presence of polyneuropathy or plasma cell proliferative disorder. Minor criteria include sclerotic bone lesions, organomegaly, edema, endocrinopathy, papilledema or skin changes [2]. Polyneuropathy is a predominant feature of POEMS syndrome and is found in >90% of the cases. It is usually a sensorimotor, axonal and demyelinating type polyneuropathy [3]. As in our patient, both axonal and demyelinating polyneuropathy are seen on electrodiagnostic studies. The mechanism of neuropathy is not known but the recent evidence of the presence of anti-neural antibodies points to an immunological mechanism [4]. Endocrinopathies occur with a frequency of 60%-80% and the most common are gonadal failure (70%) and glucose intolerance/diabetes mellitus (50%). Hypo or hyperthyroidism, hyperprolactinemia and adrenal insufficiency have also been reported. The mechanism of endocrinopathy is also not obvious; however, involvement of direct acting antibodies against hypothalamo-hypophyseal-axis and endocrine end organs has been hypothesized [5]. In our patient, impotence, loss of libido and testicular atrophy and hypothyroidism on ancillary laboratory investigation were present. Increased levels of vascular endothelial growth factor (VEGF) are found in POEMS syndrome. VEGF increases microvascular permeability, thereby inducing edema, ascites and pleural effusions as were present in our patient [6,7]. However, measurement of VEGF level in ascites was not available in our patient. Papilledema may be seen in approximately 37% of patients and is not associated with the increase in intracranial pressure. The real cause of papilledema is not still known. Hepatomegaly may be seen in up to 50% of patients with splenomegaly and lymphadenopathy occurring less often. The hyperpigmentation over the face, legs and hands was also observed in our case. The skin changes usually observed in POEMS syndrome are hyperpigmentation, lichenification, hypertrichosis, sclerodermoid changes and glomeruloid hemangiomas. Skin biopsy may show inflammation, fibrosis, or nonspecific changes. Monoclonal protein is detected in more than 90% of patients and may become positive in the follow- up of patients who have no monoclonal gammopathy initially [8]. Nearly all cases reported in the literature show lambda positivity as in our patient. It may be rarely found in urine and cerebrospinal fluid (CSF). The natural course of POEMS syndrome is chronic, with a reported median survival for a decade (8-13.8ys). The morbidity depends on the extent and number of systems involved. The cause of POEMS syndrome is still unknown. It is tempting to incriminate the presence of lambda light chains in the pathogenesis because of their unexpected frequency (more than 95% of patients), but histopathologic review of affected organs and nerves does not support that it is a form of deposition disorder. Increased levels of cytokines IL-1ß, TNF-Ã ¡ and IL-6, more specifically VEGF, appear to play a pathogenic role in the disorder [9,10]. In view of the constellation of a mixed polyneuropathy, monoclonal gammopathy, osteosclerotic myeloma, extravascular volume overload, bilateral papilledema, skin changes and endocrinopathies, our patient was diagnosed as POEMS syndrome. However, other close differential diagnosis like tuberculosis and hemochromatosis were ruled out with appropriate investigations. Patient was treated with combination of alkylating agent melphalan and corticosteroid. To conclude, when a patient present with unexplained sensorimotor polyneuropathy, signs of extravascular volume overload and evidence of other system involvement, a high index of suspicion should be kept for a diagnosis of POEMS syndrome, to avoid missing this rare syndrome, which is amenable to treatment. One ethical issue to which I became aware of while in college mainly involved cases illegal drugs, bribery, and cheating. Although there is an existing law against illegal drugs and most schools and universities are very stringent in imposing penalties for illegal activities and other things that are criminal in nature, sadly there are still some students who find ways to break those rules, especially outside the campus. While I am not the one involved in these illegal activities, I felt that I had a moral obligation because two of my classmates were involved. These classmates of mine were actually good people, but unfortunately, they ended up with the wrong company. It took place not very long ago, during a house party held by one of the rich students in the university. This certain student, whom I personally know, constantly held parties in his/her lavish house. My two classmates were not invited because the host of the party didn’t like their introvert nature. They were very shy, had very little friends, and were only interested in playing video games and the like. Nevertheless, as I have mentioned above, they were people who possessed values.
